How does a Co-Pay differ from Co-Insurance?

Both terms refer to the amount of the charges that are due from the patient.  A Co-Pay is usually a set dollar amount for each visit (i.e. $15, $20, $25, etc.), whereas a Co-Insurance is generally a percentage of the allowed amount (10%, 20%, etc.) that is due from the patient.
